Family Full (Tennis and Swim) $1,140 per year (can be paid in 12 $95 monthly payments)
Family Swim Only $780 per year (can be paid in 4 $195 monthly payments)
Family Tennis Only $660 per year (can be paid in 12 $55monthly payments)
Individual and Benefactor & Social
(individuals must be an adult for full membership or swim only).
Individual Full (Tennis and Swim) $720 per year (can be paid in 12 $60 monthly payments)
Individual Swim Only $540 per year (can be paid in 4 $135 monthly payments)
Individual Tennis Only $330 per year (can be paid in 12 $27.50 monthly payments)
Individual Volleyball Membership $100 per year
Pay As You Play Season Tennis Only $50 per USTA season (includes 1 USTAseason) Each time you play outside the league you pay a $5 court fee.
Pay As You Play Year Tennis Only $100 per year (includes 2 USTA seasons and pay as you play privileges during the Summer)
Each time you play outside the league matches you pay a $5 court fee.

Automatic Renewal: Memberships will automatically renew at the end of the contract term if the member does not notify the club, in writing, of his/her desire not to renew no later than 1 month after the end of the contract term. Memberships renewed automatically will be upon whatever terms and conditions are in effect during that contract year. Member expressly authorizes club to continue debiting monthly installments on all renewals, including automatic renewals, or to pay Annual Dues in full within 30 days.

Membership Agreement
USE OF FACILITIES: I understand that my membership is subject to approval by the Board of Directors and that membership grants me the right to use the facilities included in my membership selection. All facilities are available during operating hours as posted at the Club and in newsletters. I understand that I will receive a package including Club rules and membership directory and that it is my responsibility to abide by all rules as stated.
*MEMBERSHIP COMMITMENT: I understand that my request for membership, once accepted, is a commitment for one year and is not refundable. I agree to be responsible for all charges incurred by my guests and me. I agree to have my bank drafted monthly, my credit card charged monthly, or pay annually. I agree to provide the Club with any change of mailing address or phone number. I understand that the Club does NOT offer monthly memberships, only annual memberships with monthly payment options. Upon my signature on this contract, I agree to pay the annual dues amount for my membership category IN FULL even if I decide to terminate my membership before the end of this contract term.

MEMBER IDENTIFICATION: The Club may issue membership identification. The Club may require such membership Identification to be carried at all times while using the Facilities.
GUEST FEE: Members are responsible for registering their guests. Guest fees may be charged to member’s monthly invoice. Each guest is limited to four (4) visits per year.
BENEFACTOR: Benefactors are members who are 65 and over with no kids at home. Benefactors may use the pool up to 4 times with there family through out the year. After that they must pay $5 per person each additional visit. Benefactors do not have unlimited privledges to the pool they can only swim during business hour times. Benefactors are invited to all social functions. Benefactors may rent the pool or clubhouse for the current member prices.
INACTIVE STATUS: In the event that a member in good standing suffers from a debilitating or disabling illness or injury, as acknowledged in writing by a physician, or has other circumstances which prevent the member’s use of the Facilities for an extended period of time, the member may submit a written request to the club to have his or her membership placed on inactive status and extended for a period of up to six (6) months. The Board of Directors and
Management will evaluate the request and provide a written answer to the member.
TRANSFER OF MEMBERSHIP: Memberships are not transferable or refundable. Any extenuating
circumstances require a written explanation submitted to the Club’s General Manager.
ADVERTISEMENTS, SOLICITATION AND PRIVACY: The roster list of Creekside Tennis and Swim
Club members will be updated and distributed annually. This list shall not be used for any purpose other than official Club business and recreation. No person shall originate, solicit, circulate, or post petitions in or around the Club
Facilities without the prior approval of the Club General Manager.
ASSESSMENTS: Members are subject to assessments as may be determined by the Board. The Board of Directors will seek other reasonable funding options before deciding to impose an assessment. However, the nature of this type of club may mean member assessments may be necessary to sustain club operations.
ASSOCIATED RISKS: As a condition of using the Facilities, I agree to assume all risks associated with the use of the Facilities and agree to release and indemnify the Club from and against any and all claims and losses of every
kind and character for death, personal injury, property damage, or any other liability, damages, fines, or penalties, including costs, attorneys’ fees and settlements, whether or not based on the acts or omissions of Club operations, resulting from, arising out of or in any way connected with the use of the Facilities by my family, my guest, me, approved designees, and their family and guests, except to the extent that the same are the direct result of the gross negligence or willful misconduct of the Club or its employees. As used in this paragraph, “the Club” shall include
Creekside Tennis and Swim Club and its members, and the heirs, successors, assigns, officers, directors and employees of Creekside Tennis & Swim and its members, all persons, and entities with which it is or may in the future become affiliated. This paragraph shall survive the termination of this agreement with respect to any property damage, personal injury, or death occurring prior to such termination.
